- Preferred appointment day/time. (hours are listed below)
- Describe the nature of your appointment. (e.g., check up, cleaning, toothache, ect. )

Once an appointment is made, we reserve a spot just for you. Because of this we like to call you a day or two in advance to confirm your appointment. If you are unable to make your appointment for any reason, we ask that you please call to let us know as soon as possible. We understand that this happens on occasion and we can reschedule you much more effectively if given the opportunity.
